Hospital Aftercare / Prevention of Admission

The Hospital After Care / Prevention of Admission Service is tailored to individual needs and provides practical and emotional support for a temporary period to:-

  • help people manage again at home following discharge from hospital
  • prevent the need for admission to hospital.

Support visits are usually made once or twice per week, depending on need, for a maximum of 6 weeks, to help regain confidence and independence.

Who can make referrals?

  • Nursing staff
  • Carers
  • Family or friends
  • People who have had previous experience of the service often refer themselves.

What does the service provide?

  • Visits to the ward (where appropriate) to establish requirements prior to discharge
  • Help to ensure there is essential food on arrival home from hospital
  • Help with light meals and drinks
  • Shopping
  • Collecting pensions and prescriptions
  • Light household tasks
  • Escorting to the shops etc
  • Making laundry arrangements

The service is not intended to substitute other home care services. These will continue, if they are already in place, alongside the Age Concern service.
